Intake Process
How It Works
Our admissions process is designed to be clear, respectful, and centered on finding the right fit for each individual.
Initial Contact
Reach out to our team by phone or email. We welcome calls from families, DDD caseworkers, Support Coordinators, and individuals themselves. We'll answer your questions and discuss whether our services may be a good fit.
Referral Submission
A formal referral is submitted through the individual's DDD Support Coordinator. We work closely with DDD to ensure all documentation and authorizations are in order before moving forward.
Assessment & Planning
We conduct an intake assessment to understand the individual's needs, goals, preferences, and support requirements. This helps us develop a personalized support plan that reflects who they are.
Placement & Transition
Once a placement is confirmed, we work with the individual, their family, and their support team to ensure a smooth and comfortable transition into our program. We are with you every step of the way.
